The Vision for Life Plan reflects American Vision Partners’ (AVP) commitment to supporting eligible LASIK and PRK patients beyond their original procedure.

What is the Lifetime Vision Plan?

The Vision for Life Plan is an optional enhancement plan available to eligible LASIK and PRK patients at Southwest Eye Institute or at any American Vision Partners brand. If your vision changes due to nearsightedness, farsightedness, or astigmatism and an American Vision Partners surgeon determines that a laser enhancement is medically appropriate, eligible Vision for Life Plan members may receive that enhancement at no additional cost.

This plan is available only for qualifying LASIK and PRK patients and must be purchased before your original laser vision correction procedure.

Vision for Life Eligibility Requirements

Eligibility is based on your individual eye health, prescription, corneal measurements, and other clinical factors. Enrollment in the plan does not guarantee that a future enhancement will be appropriate or medically recommended. To enroll in and maintain eligibility for the Vision for Life Plan, patients must meet the following requirements:

Purchase the Vision for Life Plan before LASIK or PRK procedure

Have their original LASIK or PRK procedure performed by an eligible American Vision Partners surgeon

Meet the required prescription and eye health criteria before surgery

Have vision that is correctable to 20/30 or better before laser vision correction

Complete all required follow-up visits during the first 12 months after surgery

Remain current on all payments related to the original laser vision correction procedure

Complete an annual eye exam each year after surgery with a qualified eye care provider

Return for evaluation by an American Vision Partners provider if vision changes occur

What Vision for Life Does Not Cover

The plan does not cover vision changes caused by cataracts, glaucoma, retinal conditions, eye injuries, other eye diseases, presbyopia, or other eye procedures.

It also does not cover glasses, contacts, medications, travel, missed work, or enhancements performed outside the American Vision Partners network.

Enrollment in the Vision for Life Plan

Eligible patients may enroll in the Vision for Life Plan before their LASIK or PRK procedure at Southwest Eye Institute or at an AVP network provider.

The one-time enrollment fee is $275 per eye, for a total of $550. Your surgeon or a member of the vision correction team will review the plan details, eligibility requirements, and enrollment paperwork with you before surgery.

You will have an opportunity to ask questions, understand the plan terms, and decide whether the Vision for Life Plan is right for you before your procedure.

How the Vision for Life Plan Works

If you are an eligible LASIK or PRK candidate, you may choose to enroll in the Vision for Life Plan before your procedure. Your Southwest Eye Institute team will explain the requirements and answer any questions you have.

Your surgeon performs your personalized laser vision correction procedure and guides you through your recovery and scheduled follow-up care.

Complete your required post-procedure visits, remain current on related payments, and schedule an annual eye exam each year after surgery.

If you notice a meaningful change in your vision over time, schedule an evaluation with an American Vision Partners provider. Your provider will assess your prescription, corneal health, and overall eye health.

When a laser enhancement is medically appropriate, and all Vision for Life Plan requirements are met, the enhancement procedure may be performed at no additional surgical cost.

Vision for Life Plan FAQs

No. Most LASIK and PRK patients do not need a future enhancement. However, vision and prescriptions can naturally change with time, which is why some patients choose the Vision for Life Plan before their procedure.

No. A future enhancement depends on your eye health, corneal thickness, prescription, and other medical factors at the time of evaluation. Your surgeon must determine that the procedure is medically appropriate.

No. The Vision for Life Plan does not cover presbyopia, the normal age-related change that makes close-up vision more difficult. Many people eventually need reading glasses as they age, even after LASIK or PRK.

No. The Vision for Life Plan must be purchased before your original LASIK or PRK procedure.

You must have an annual eye exam with a qualified provider and return to an American Vision Partners provider if you experience vision changes and want to be evaluated for a possible enhancement.
